Minimally invasive surgery has required the acquisition of new skills on the part of surgeons and the creation of new tools, an area in which robotics is an invaluable help. The current trend is towards reducing the number of incisions practiced on the patient to a minimum. SILS (Single Incision Laparoscopic Surgery) and NOTES (Natural Orifice Transluminal Endoscopic Surgery) techniques have thus emerged, and these require a new generation of robotic tools.

La actual tendencia se dirige a la reducción de las incisiones que se practican en el cuerpo del paciente al mínimo. De este modo, aparecen las técnicas SILS (Single Incision Laparoscopic Surgery) y NOTES (Natural Orifice Transluminal Endoscopic Surgery), donde se requieren una nueva generación de herramientas robóticas.

MATES addresses the subject of developing new robotic tools that are useful for both of the previously mentioned techniques and that cover both the preoperative and the intraoperative periods. Specifically, it deals with the design and development of a robotic platform capable of placing a micro-robot equipped with sensors and surgical tools within the abdominal cavity to work collaboratively with the surgeon. The system will be programmable off-line to establish a master plan for use during the operation and in intraoperatively via a human-machine multimodal interface. On the other hand, the robotic system itself consists of two positioner arms and a micro-robot.  The two arms, by use of magnetic fields, will move and position the robot in the operative field inside the abdomen. As a configuration of one of the robotic positioner arms, we have considered a hyper-redundant structure that will hold the micro-robot at one end for entry into the abdominal cavity. The micro-robot will be able to release itself from the hyper-redundant structure, and with the aid of the other positioner arm, which will have a standard configuration, it will be moved around the abdominal wall with the use of a magnetic field. The micro-robot will be able to unfold small arms on which cameras, touch sensors, light sources or small surgical tools will be positioned.

During the development of this proposal, techniques for an intraoperative simulator will be addressed that incorporate models based on real data from the operative field, as well as the possibility of updating this information during surgery; this will serve as the basis for the human-machine multi-modal interface mentioned above. In order to integrate all of the system elements, strategies will be developed for planning and control of the movements of both the two robotic arms and the micro-robot in response to the off-line programming of the surgeon and his commands during surgery. Both the direct teleoperation and the performance of automatic actions that collaboration with the surgeon involves are taken into account. Finally, to verify the work done, a series of in-vitro experiments is anticipated.

MATES pretende abordar el desarrollo de nuevas herramientas robóticas útiles para los dos tipos de técnicas nombradas anteriormente y que abarquen tanto el ámbito preoperatorio como el intraoperatorio. En concreto, se acometerá el diseño y desarrollo de una plataforma robótica capaz de situar en el interior de la cavidad abdominal un micro-robot dotado de sensores y herramientas quirúrgicas que trabaje de forma colaborativa con el cirujano. El sistema podrá programarse fuera de línea para establecer el plan maestro durante la intervención y de forma intraoperatoria mediante un interfase persona-máquina multi-modal. Por otro lado, el sistema robótico en sí, constará de dos brazos posicionadores y un micro robot, de suerte que los dos primeros, mediante el uso de campos magnéticos, desplazarán y ubicarán al tercero en el campo operatorio del interior del abdomen. Como configuración de uno de los brazos robóticos posicionadores, se ha considerado una estructura hiper-redundante que sostendrá en su extremo al micro-robot para introducirse en el interior de la cavidad abdominal. El micro-robot podrá desprenderse de la estructura hiper-redundante, y mediante el otro brazo posicionador, que tendrá una configuración estándar, se moverá desde el exterior de la pared abdominal mediante el uso de un campo magnético. El micro-robot podrá desplegar pequeños brazos en los que se ubicarán cámaras, sensores de tacto, fuentes de luz o pequeñas herramientas quirúrgicas.

De esta forma, en el desarrollo de esta propuesta, se abordarán técnicas para la realización de un simulador intraoperatorio que incorpore modelos basados en datos reales del campo operatorio, así como la posibilidad de actualizarlos durante el transcurso de la intervención, que servirá como base para el interfase persona-máquina multi-modal mencionado anteriormente. Con el objeto de integrar todos los elementos del sistema, se elaborarán estrategias de planificación y control de movimientos que impliquen la coordinación de los dos brazos robóticos y del micro-robot, en respuesta a la programación fuera de línea del cirujano y a sus órdenes durante la intervención. Se contempla tanto la teleoperación directa como la realización de acciones automáticas que impliquen una colaboración con el cirujano. Finalmente, para verificar todo el trabajo realizado se prevé la realización de una serie de experimentos in-vitro.
